KINDERGARTEN WELCOME PACKAGE
Once you have submitted the online registration form you will be contacted by the school to provide the following information: Proof of Birth (birth certificate or passport) and Proof of address (current utility bill or agreement to purchase or lease).

KINDERGARTEN INTAKE FORM: You will be contacted over the summer and be given an intake appointment for you and your child to attend in September prior to classes beginning. In preparation for this appointment please complete this form in late August to capture the most up to date information about your child and bring it with you to your scheduled intake meeting with your child’s teacher. IMMUNIZATION: Records are kept by Ottawa Public Health - you are required to enter the immunization information directly with them, this can be done by following the link below:

EXTENDED DAY PROGRAM: This a fee based program that provides care for students before and/or after school. More information is provided at the following link:
Extended Day Program
Please notify the school office if your child is planning to attend the EDP program. We will coordinate a start date for a smooth transition.
TRANSPORTATION: You can inquire about transportation at the time of registration. More detailed information can be found at:
Kindergarten Transportation Information
If you qualify for a school bus, the information will be available online by mid August. The routes have been set up using the information that you provided the school when you registered, if that has changed you need to contact the school to update the information. Changes take 3-5 days to process.

ATTENDANCE: We practice a “safe arrival” program at CMPS - if your child will be absent or late for any reason, Please complete the online ATTENDANCE FORM before 8:30 am.
On the Chapman Mills PS Homepage, click the “STUDENT ATTENDANCE" icon on the left hand side and complete the form.
